CASE MANAGER Apply for this Position

The Case Manager acts as a liaison for patients with the referral network. The Case Manager assists patients, in agreement with their counselor, to seek social, vocational and rehabilitative services on an as needed basis. This is a full-time, benefits-eligible position at our Brockton, MA program, located at 180 Centre Street. The scheduled hours for this position are Monday through Friday 6 am to 2 pm. The pay rate for this job is between $11-$14 per hour.
He or she will help to assesses, analyzes and determine patients’ needs and make the most appropriate referral. This person will be responsible for accurately maintaining referral network paperwork; documenting all patient/referral contact and submitting appropriate activity logs; and maintaining an accurate record system.
The person in this role assists in coordinating and documenting the intake process and maintains 15 hours of face-to-face contact. He/she is responsible for reporting all pertinent issues to appropriate staff/supervisor and giving timely feedback to the appropriate staff. The Case Manager also completes all MIS information, trips, transfers and transportation forms as requested.
We are looking for someone who exhibits a team-oriented approach and treats patients and co-workers with respect and courtesy. To excel in this role, the individual must be personally committed to excellence in serving patients and exhibit compassion while consistently maintaining professional boundaries and providing a friendly, therapeutic environment.
The qualified candidate should have a Bachelors Degree in the Human Services, Psychology, Social Work, or related field of study. He/she must have exceptional assessment and interpersonal skills and must possess strong written and verbal communication skills in English. Bilingual Spanish helpful but not required. Prior knowledge of and experience with substance abuse treatment preferred.

We are looking for an Office Assistant for our Boston program. This is a full-time, benefited position scheduled for 37.5 hours per week. The schedule will be Monday through Friday 6a.m. to 2 p.m. This role will report to the Office Manager and will be expected to provide intermediate-level clerical support to our clinical team and manage clinical reception area.
The Office Assistant will be responsible for reception area coverage and greeting patients and guests to the program. He or she will answer the telephone, direct calls, assist with scheduling, and answer client questions as appropriate. The job will also include filing and basic administrative tasks.
Successful candidates should have at least two years of solid administrative experience, preferably in a high-volume health care setting. They should be known for being outgoing yet extremely tactful, and customer-focused. They must possess exceptional interpersonal and communication skills.
The Office Assistant job requires an individual to be professional, organized, and able to multi-task. Our busy environment allows for a great deal of learning and professional development; however, it requires the ability to take initiative, learn quickly, and organize and prioritize one's own work. Strong customer service and phone skills are a plus.
