- Calling any Habit OPCO program in their area.
- Walking into any Habit OPCO program and beginning the process with program staff.
- Obtaining referrals from Acute Care Settings or Primary Care Physicians.
Intake staff will determine eligibility for services and appointments for intake will be scheduled in a timely manner. If an individual
does not meet admission requirements, Habit OPCO staff will make recommendations for alternative placements and/or provide referral resources.
Habit OPCO does prioritize the following populations for admission to our programs: pregnant women, persons with HIV/AIDS and referrals
from acute care settings.
